Complete this form for a Certificate of Insurance for the location(s) provided for club/team practices and/or events.
Certificates are only good for USAV-sanctioned clubs for tryouts, practices, clinics and tournaments when all participants are USAV registered. No fee is required, but the Badger Region office is REQUIRED to have on file specific information on the use of each facility.
In addition to this request form (one request per facility), please send an excel spreadsheet or word document showing the site name, practice dates and times that these facilities will be used.
